We provide high quality medical, dental, and behavioral healthcare to all, regardless of ability to pay.

Open Health Care Clinic accepts all patients regardless of their insurance or financial status. We are a federally funded health center which in turn allows us to offer a wide range of services to patients through a sliding-fee scale.  

We offer a sliding-fee scale to eligible patients based on annual household income for most of our services. Income guidelines and acceptable verification for the sliding-fee scale are as listed below.


How Do I Qualify For Discounted Fees?

Patients applying for the sliding fee discount program must provide written verification of income and family size.

Below Are Examples of Documents Needed to Show Proof of Income:

  • ✔Public assistance
  • ✔Veterans' Payments or Survivor Benefits

  • ✔Interest, Dividends, Rents, Royalties, Income from Estates, Trusts
  • ✔Educational Assistance
  • ✔Alimony or Child Support
  • ✔Assistance from outside the household
  • ✔Other miscellaneous sources.
