THE KAISER PERMANENTE
HEALTH CARE CAREER SCHOLARSHIP PROGRAM

Kaiser Permanente, in partnership with Oregon Health Career Center is pleased to announce a new scholarship program for students who dream of a career in the health care field.

• Up to 115 scholarships are available.
• Scholarships awarded in the amount of $2,000.
• Schools must be in the Kaiser Permanente Northwest service area.
• Applicants must be high school seniors during the 2008-2009 school year.
• Scholarships are driven by teacher recommendations.
• Applications must be completed and postmarked no later than Friday, January 9, 2009.
• Scholarship award winners will be announced in early May 2009.

Teachers: To nominate a student, please download the application below, fill it out, and submit it by Friday, January 9, 2009 via fax (503-682-1311) or mail (Oregon Health Career Center, Attn: Sara Diaz, 25195 SW Parkway Ave. #204, Wilsonville, OR 97070). Applicants MAY NOT submit teacher nominations with the application packet.
