Non-Medical Case Manager

The position is responsible for assisting clients in obtaining social, community, legal, financial, and other needed services.

Duties:

  • Identify, compile and review any necessary assessments and other documents that are relevant to the client needs.

  • Assist the client to initiate, develop and maintain a support network and to meet the recipient’s individual needs.

  • Assist the client to overcome obstacles, recognize potential opportunities, and develop creative opportunities.

  • Assist in arranging professional evaluations and other components of the assessment to develop strategies for obtaining the services, resources and supports needed to achieve the recipient’s personal outcomes.

  • Be knowledgeable of potential community resources, including formal resources such as Food Stamps, SSI, housing, etc.

  • Ensure client’s choice.

  • Follow up with the client on a regular basis.

  • Assist the client to use the findings of formal and informal assessments to develop and implement support strategies to achieve personal outcomes.

  • Assist and remove any barriers that the recipient may have in obtaining the identified services.

  • Keep the required documentation in the record.

  • Complete 24 hours of annual training.

  • Perform related duties as assigned.

Qualifications:

  • An associate’s degree from an accredited college or university preferably in a human service-related field.

  • A minimum of one year paid work experience with persons with HIV/AIDS or other catastrophic illness and/or Sate or National certification from a recognized state/national certification organization and/or licensing organization.

  • Knowledge and training in assessment of needs and care plans.

  • Case management experience.

Executive Assistant

Position is unique in its reporting relationship, requirements for job skills and level of experience and the independent judgment needed to successfully accomplish multiple responsibilities in a professional manner. Coordinates activities of the Board of Directors and the CEO in accordance with the corporation's Articles of Incorporation, bylaws, policies, procedures, mission and vision. Supports CEO.

Minimum Education:
Completion of office education courses focusing on organizational and administrative practices. Prefer training in accounting at the college level.

Minimum Work Experience:
Requires a minimum of three years’ experience in positions providing support for executive level personnel within a non-profit agency. Requires advanced computer skills in Microsoft Access, Excel, Outlook, PowerPoint, Project and Word.

Job Knowledge:
Possess knowledge of organizational, administrative, and supervisory practices and principles and of accounting and budgeting procedures. Requires skills in managing multiple responsibilities in accordance with established timelines; in planning, analyzing and coordinating activities; in verbal and written communication; in problem solving and decision making. Requires the ability to work independently and maintain the confidentiality of information.

Internal Physician

Responsibilities:

The Internal Medicine Physician provides comprehensive medical services as a part of our expanding primary care team. The physician provides patients with high-quality treatment and diagnostic services. The physician is responsible for meeting, assessing and diagnosing patients in the primary care setting and providing healthcare services that meet state and national standards.

Accepts responsibility for ensuring that his/her practice and conduct meet the standards of the professional, ethical and relevant legislated requirements

Clinical Management

  • Work with the primary healthcare team to provide patients with primary medical care services within the standard of care for Internal Medicine patients.

  • Order laboratory, radiology and diagnostic tests and refer patients to specialists when appropriate.

  • Analyze reports, test results, medical records and examinations to diagnose condition of patients and propose treatment options.

  • Prescribe pharmaceuticals, other medications, and treatment regimens to treat identified and documented medical conditions.

  • Prescribes vaccinations to immunize patient from communicable diseases.

  • Refers patients to medical specialist for consultant services when necessary for patient’s well-being.

  • Documents the patient’s visit including medical history, physical exam, diagnoses and plan of action in the electronic health record.

  • Provides 50% effort on the treatment and management of diabetes for health center patients.

  • Provides 25% effort on general primary care services.

  • Provides 25% effort in the treatment and management of HIV to newly diagnosed and non-virally suppressed health center patients.

Other Duties

  • Adhere to departmental policies, procedures and objectives

  • Participates in ongoing quality improvement objectives and safety, environmental, and infection control standards.

  • Maintain patient confidentiality and comply with all federal and state health information privacy laws.

  • Record complete, timely and legible medical and billing records.

Minimum Qualifications:

  • Current license to practice as a physician in Louisiana

  • Current Drug Enforcement Agency (DEA) certificate

  • BLS for healthcare provider is required
